Leider kannst du HTML-Formularelemente wie <input> nicht direkt in MathML einbetten – der Standard erlaubt nur MathML-Elemente innerhalb von <math>.
Wenn das nicht erlaubt wäre, stünde es nicht mit einem Beispiel in der Spezifikation §2.2.1.
und es funzt in Chrome, Edge und Firefox!
